Hearts For Dementia Home Care Founder and CEO, Lolita Tramel has been on the frontlines for over a decade, providing personalized care and support to patients and families confronted with serious illnesses. Lolita serves as a dual board-certified Family Nurse Practitioner, Psychiatric Mental Health Nurse Practitioner, along with holding a multitude of other certifications, including as a Certified Senior Care Manager, a Certified Dementia Practitioner, a NCCDP Certified Alzheimer’s Disease & Dementia Care Trainer, a Certified Montessori Dementia Care Professional, a Certified Dementia Support Group Facilitator, and as a PAC Certified Independent Trainer for Teepa Snow.
Through personal tragedy and pain, while caring for her dear mother battling Alzheimer’s Disease, Lolita was inspired and compelled to launch Hearts For Dementia Home Care, with a special focus set on dementia care. Her clinical expertise and trustworthy nature, combined with her experience as an NCCDP Alzheimer’s and Dementia Care Trainer and PAC Certified Independent Trainer for Teepa Snow, are the foundations for a wealth of Home Care knowledge.
After developing many critical skills in the fields of Family Medicine and Neuropsychiatry while diagnosing and treating patients, she has positioned herself to be able to deliver quality, personalized care plans based on each person’s circumstance, history, and medical needs. Lolita truly believes in a people-first approach to care and works collaboratively with each person’s care team to meet the different needs of all of her valued clients.
